  • Your insurance card
  • Physician referral forms, if required by insurance
  • List of prescriptions and/or over-the-counter medication, including dose and frequency
  • Information about your medical and surgical history
  • Recent x-rays or relevant records
  • A change of clothing that can easily expose the body part that
    requires treatment.

What to expect

Each new patient will receive a thorough one on one individualized initial evaluation by a licensed PT, which will take approximately 60 - 90 minutes. For your privacy and convenience, upon every visit you will be provided a private room. All our patients are seen promptly with minimal to no waiting time. All subsequent visits are one on one and around 45 minutes to 90 minutes in length. Our hands on treatments consist of manual therapy, joint mobilization, myofacial release, muscle relaxation techniques, therapeutic exercise, pain and inflammation reducing modalities, and patient education.

My orthopedic surgeon recommended Paul LaRosa and Empire PT without
hesitation after I suffered a stage IV shoulder separation in a bicycle
crash. Only after using Empire did I understand why my surgeon was so
confident in his recommendation.

After working with Paul and Empire I have to say it was the best PT
experience I have had, ever (I’ve been to 4 other groups). Having
personally been tended to by Paul, my opinion is that he has mastered sports
rehab. Paul is switched on, motivated to help, and runs a great office
(Sally his office manager is a on point with billing Q’s and appointments).

Beyond this, Paul’s charisma, and professional manner are just plain 1st
class… one of the most positive and motivational guys you could meet.
What impressed me in particular about Paul was that he approached my problem in a very open-minded and intuitive way. First, he studied the specific demands on the shoulder for a bike racer. Then Paul had me ease into several different dimensions of the rehabilitation in a very systematic way. After a few months of hard work with Paul I had achieved my rehab goals and was back racing bicycles and was even able to sprint for a victory in the last race of the season.
